BLOODCHEK MEDICAL AND ITS AFFILIATES SELL PROFESSIONAL MEDICAL DEVICES BASED UPON BUYERS STATED NEEDS, AND AS SUCH IT IS THE RESPONSIBILITY OF THE BUYER TO DETERMINE THE SUITABILITY OF ANY PRODUCTS FOR THEIR INDIVIDUAL NEEDS. BLOODCHEK MEDICAL AND ITS AFFILIATES MAKE NO CLAIM AS TO THE ACCURACY OF ANY TESTING PRODUCTS. THE BUYER SHOULD DETERMINE IF ANY MEDICAL DEVICE SOLD BY BLOODCHEK MEDICAL WILL WORK FOR THEIR INTENDED PURPOSE. WE WILL PROVIDE ANY ASSISTANCE WE CAN IN THE CHOICE OF EQUIPMENT, HOWEVER WE MAKE NO SUITABILITY CLAIMS WHATSOEVER FOR ANY PRODUCTS WE SELL.

WARRANTY

CARDIOCHEK OR CHOLESTECH EQUIPMENT SOLD BY BLOODCHEK.COM COME WITH A ONE-YEAR WARRANTY UNLESS OTHERWISE INDICATED. OUR WARRANTY STARTS THE DAY YOU RECEIVE YOUR EQUIPMENT. OUR LIMITED WARRANTY COVERS THE ELECTRICAL AND MECHANICAL FUNCTION OF THE ANALYZERS AND PRINTERS WE SELL. WE DO NOT COVER EQUIPMENT THAT HAS BEEN DAMAGED OR SHOWS SIGNS OF PHYSICAL ABUSE IN ANY MANNER OR HAS GOTTEN WET AT ANY TIME.  IF YOU ARE SENDING ANY ITEMS BACK TO US FOR ANY REASON, WE DO NOT WARRANTY EQUIPMENT THAT ARRIVES AT OUR OFFICE IMPROPERLY PACKED FOR SHIPMENT. IT IS THE SOLE RESPONSIBILITY OF THE CUSTOMER TO ENSURE THAT ANY MEDICAL EQUIPMENT SENT BACK TO US IS PACKAGE PROPERLY. IF ANY EQUIPMENT IS SENT TO US IMPROPERLY PACKAGED - WE WILL CHARGE A SERVICE CHARGE TO CORRECT ANY ISSUES CAUSED BY IMPROPER PACKING/SHIPPING. IF ANY IMPROPERLY RETURNED EQUIPMENT CANNOT BE REPAIRED, THE WARRANTY IS VOID. BLOODCHEK MEDICAL SHALL BE THE SOLE DETERMINER OF CAUSE OF DAMAGE. IF YOU HAVE ANY QUESTIONS ON HOW TO PROPERLY PACKAGE ANY EQUIPMENT FOR RETURN - CONTACT US BEFORE YOU SHIP IT TO US. PLEASE NOTE - SERVICE AND WARRANTY CUSTOMERS OUTSIDE THE 48 CONTINUOUS UNITED STATES AND INTERNATIONAL CUSTOMERS ARE RESPONSIBLE FOR ALL SHIPPING COSTS TO US FOR REPAIR AND RETURN SHIPPING TO CUSTOMER.

RETURNS

BLOODCHEK MEDICAL WILL ACCEPT RETURNS OF NEW ANALYZERS AND PRINTERS IF THEY ARE RETURNED WITHIN 30 DAYS OF RECEIPT BY CUSTOMER IN NEW, UNOPENED, AND UNUSED CONDITION. BLOODCHEK MEDICAL DOES NOT ACCEPT RETURNS OF TEST MEDIA, SPECIFICALLY TEST STRIPS OR TEST CASSETTES UNDER ANY CIRCUMSTANCES, AT ANY TIME, WHETHER OPENED OR NOT, USED OR NOT. THERE ARE NO EXCEPTIONS TO THIS CONDITION. IF YOU CANNOT AGREE TO THIS CONDITION, THEN WE CANNOT ACCEPT PURCHASES FROM YOU. BLOODCHEK MEDICAL SHALL BE THE SOLE DETERMINER OF ANY RETURNS.  

CUSTOM PACKAGES

ANY CUSTOM ITEMS OR PACKAGES RETURNED TO BLOODCHEK MEDICAL, AFTER RECEIPT BY CUSTOMER WITHIN 30 DAYS, EVEN IF NOT OPENED, SHALL BE SUBJECT TO A 35% RESTOCKING FEE. A CUSTOM PACKAGE IS ANY PRODUCT OR GROUP OF PRODUCTS THAT ARE NOT SOLD EXACTLY AS, AND/OR IN QUANTITY OF, ITEMS LISTED ON OUR WEBSITE. 

ONCE AGAIN, TEST STRIPS AND TEST CASSETTES ARE NOT ACCEPTED FOR RETURN UNDER ANY CONDITION. ALL TEST MEDIA HAVE EXPIRATION DATES - IT IS THE CUSTOMERS RESPONSIBILITY TO TRACK THIS. IF ANY REFUND IS APPROVED, ANY FUNDS ARE SENT TO CUSTOMER WITHIN 7-10 BUSINESS DAYS OF RECEIPT OF THE RETURNED ITEM IS NEW, UNOPENED, AND UNUSED CONDITION. BLOODCHEK MEDICAL ONLY SELLS TO MEDICAL PROFESSIONALS.

BLOODCHEK MEDICAL IS NOT AN AUTHORIZED DISTRIBUTOR OF QUEST, HENRY SCHEIN, OR ABBOTT PRODUCTS.
